Usually, this information can be found by logging into your online banking and locating the card under "Card History". From there, you can simply send us a screen shot. 

If your online banking does not have this feature, the next step would be to acquire a signed and stamped paper copy confirming card ownership. This can be done by visiting your bank branch.

We need to see the following:

• Name
• Last four digits of the card
• Logo
• Branch stamp
