Usually, this information can be found by logging into your online banking and locating the card under "Card History". From there, you can simply send us a screen shot. 

If your online banking does not have this feature, the next step would be to obtain a signed and stamped paper copy confirming that the card belonged to you by visiting your bank branch. 

We need to see the following: Name, last four digits, logo and branch stamp.
